Objectives: The aim of this placebo-controlled, double-blinded, randomized clinical investigation was to evaluate the influence of 8% arginine-containing prophylaxis products on pain related parameters (Schiff and VAS scores) compared to a negative control in patients from a German dental clinic suffering from severe dentine hypersensitivity (DH) over 24 weeks. Methods: After ethical approval by the Ethics Committee of the Martin Luther University Halle-Wittenberg (Germany), 101 informed patients having at least two teeth (no molars) with air blast DH scores 2, 3 (Schiff Cold Air Sensitivity Scale) were randomly assigned to either the control or test group. All were treated with a single application of the in-office paste followed by daily toothpaste use (control products without desensitizing ingredients and test products containing 8% arginine, both with the same level of fluoride).The Schiff and VAS scores were evaluated before and after application of the in-office paste and after 4, 8, and 24 weeks for both study teeth. Results: A total of 98 patients completed the study. The distribution of subjects with DH Schiff scores 2 and 3 at screening was similar in both groups. There were no statistically significant differences in the Schiff and VAS scores at baseline between test and control subjects (p>0.05). The pooled data for tooth 1 and tooth 2 yielded significantly greater pain relief as assessed by the Schiff score in the test group than the control group at any appointment, both in the t-test and Wilcoxon signed rank test analyses (p<0.05). The VAS showed no significant differences between both groups. Conclusions: Significant improvements (Schiff score, pain relief) in a German population were demonstrated after application of the in-office paste and over the 24- week period of brushing with the 8.0% arginine-containing toothpaste. Endodontisch bedingte Zahnverfärbungen gehen vom Wurzelkanalsystem aus und sind Folge von reaktiv-physiologischen (Obliteration) oder pathologischen Vorgängen (traumabedingte Einblutung, Pulpanekrose, intrakoronale Resorptionen). Gleichzeitig können auch endodontische Interventionen (Pulpaüberkappungen, Wurzelkanaleinlagen, Wurzelkanalfüllungen) bei Verwendung nichtfarbstabiler Materialien zu sichtbaren Diskolorationen führen. Häufig besteht eine ästhetisch bedingte Behandlungsindikation. Bei bestehender oder notwendiger Wurzelkanalbehandlung ist das interne Bleaching eine zuverlässige und zugleich minimalinvasive Maßnahme zur Wiederherstellung der ursprünglichen Zahnfarbe. Verfärbte obliterierte Zähne ohne Zeichen einer infizierten Pulpanekrose können zur Farbkorrektur extern gebleicht werden.

Die Versorgung subgingivaler Zahnläsionen stellt eine Herausforderung im klinischen Alltag dar. Neben der Durchführung einer chirurgischen oder kieferorthopädischen Kronenverlängerung existiert die Technik der sogenannten Stufenhebung. Hierbei wird unter relativer Trockenlegung Komposit appliziert und der Rand des Defekts in den supragingivalen Bereich gehoben. Anschlie-ßend wird der supragingivale Anteil mit einer direkten oder indirekten Restauration versorgt. Trotz der Verletzung der biologischen Breite wird der tiefe Füllungsrand bei einer gewissenhaften Überschussentfernung und guten Mundhygiene des Patienten vom Parodont toleriert. Die Durchführung der Stufenhebung ist techniksensitiv und Erfolge sind nur bei einer sorgfältigen Anwendung der Adhäsivtechnik zu erzielen. Unter diesen Voraussetzungen stellt die Stufenhebung eine konservative Behandlungsalternative zur Kronenverlängerung dar.

Purpose: This study investigated the effect of toothbrush bristle stiffness and brushing force on the cleaning efficacy in vitro.
Materials and Methods: Eighty bovine dentin samples were allocated to eight groups (n=10). Two custom-made toothbrushes of different bristle stiffness (soft and medium) were tested at four different brushing forces (1, 2, 3 and 4 N). Dentin samples were stained in black tea and brushed (60 strokes/min) for a total of 25 min in a brushing machine with an abrasive solution (RDA 67). Photographs were taken after 2 and 25 min of brushing time. Cleaning efficacy was measured planimetrically.
Results: After 2 min of brushing, the soft-bristle toothbrush did not cause statistically significantly different cleaning efficacy at different brushing forces, while the medium-bristle toothbrush cleaned statistically significantly less efficaceously only at 1 N. Comparing the two different toothbrushes, higher cleaning efficacy was observed only at 1 N for the soft-bristle brush. At 25 min brushing time, the soft-bristle cleaned statistically significantly better at 4 N compared to 1 N and 2 N and at 3 N compared to 1 N. Using the medium-bristle, cleaning efficacy increased with increasing brushing force. After 25 min of brushing, no statistically significant difference was observed between the two different toothbrushes.
Conclusion: Irrespective the brushing force, the use of a soft or medium toothbrush results in comparable cleaning efficacy. At 2 min brushing time, increasing the brushing force does not increase the cleaning efficacy.

Purpose: Antibiotics play an important role in treating periodontal diseases. Due to the effectiveness of antibiotic therapies, their usage in dentistry has significantly increased. The aim of this study focused on the in-vitro susceptibility of different gram-negative oral bacteria species – which are associated with periodontal diseases (Fusobacterium spp., Capnocytophaga spp. and Leptotrichia buccalis) and have different geographical origins (Asia and Europe) – against antimicrobials that are clinically relevant in dental therapy.
Materials and Methods: A total of 45 strains were tested (29 Fusobacterium spp., 13 Capnocytophaga spp. and 3 L. buccalis) that were either isolated from Chinese patients or were obtained from different strain collections. Their antimicrobial susceptibility to the antimicrobial agents benzylpenicillin, amoxicillin, amoxicillin-clavulanic acid, ciprofloxacin, moxifloxacin, clindamycin, doxycycline, tetracycline and metronidazole was tested using the E-Test. Strains with particular resistance to penicillin, clindamycin and metronidazole were further analysed for resistance genes.
Results: All tested bacterial isolates were sensitive to amoxicillin, amoxicillin-clavulanic acid, doxycycline and tetracycline, but showed variable sensitivity towards other antibiotics such as benzylpenicillin, ciprofloxacin, moxifloxacin, clindamycin and metronidazole.
Conclusion: The results of the present study suggest that certain periodontal disease-related bacterial strains can be resistant towards antimicrobial agents commonly used in adjuvant periodontal therapy.

Purpose: To investigate the effect of the supplementation of energy drinks with green tea extract on abrasive and erosive dentin wear.
Materials and Methods: Six groups, each comprising 15 bovine dentin samples, were prepared, yielding a total of 90 samples. Erosion was performed by immersing the samples in Red Bull and Red Bull light with and without green tea extract. Tap water with and without green tea extract was used as the control groups. The samples were subjected to abrasive/erosive cycling for five days. The following cycling was performed daily: toothbrush abrasion (20 brushstrokes; 2.5 N); eight erosive cycles (2 min storage in the respective solutions); in between the erosive cycles, storage in artificial saliva (60 min) and again toothbrush abrasion (20 brushstrokes; 2.5 N). During the night, samples were again stored in artificial saliva. Abrasive/erosive dentin wear was measured using a stylus profilometer (µm, accuracy = 40 nm). The measured dentin loss results from the vertical position shift on the y-axis from base to final profile after the wear process in 2D. Pairwise comparisons between the groups were carried out using Wilcoxon signed-rank test.
Results: The following dentin wear (median [IQR]) was measured: Red Bull: 1.9 µm (0.5); Red Bull Light: 1.3 µm (0.3); Red Bull with green tea extract: 0.8 µm (0.3); Red Bull Light with green tea extract: 0.3 µm (0.5); Tap water with green tea extract: -0.2 µm (0.7); Tap water: -1.0 µm (1.2). The comparison of all tested groups to each other proved to be statistically significant (p < 0.05).
Conclusion: The supplementation of energy drinks with green tea extract provide a protective effect against erosive/abrasive wear in vitro.

Purpose: To investigate the absolute wear caused by toothpastes with highly discrepant REA (Relative Enamel Abrasivity) and RDA (Relative Dentin Abrasivity) values on both enamel and dentin: Candida Peppermint (CP; REA: 1; RDA: 42), Colgate Total Original (CTO; REA: 4; RDA: 100), Signal White System (SWS; REA: 8; RDA: 143), and Candida White Diamond (CWD; REA 244; RDA: 12).
Materials and Methods: Eighty (80) bovine enamel samples and 80 dentin samples were divided into four groups each (n = 20) and investigated after a 6-h brushing procedure (21,600 cycles, 60 cycles/min, load of 2.5 N) with the four toothpastes. The abrasive enamel and dentin wear were registered using a contact profilometer. The median and interquartile range (IQR) of the abrasive enamel and dentin wear were calculated for each group. Pairwise comparisons were conducted using the Wilcoxon signed-rank exact test, and the p-value was adjusted according to Holm (statistical significance set at 0.05).
Results: CWD led to the highest abrasive enamel wear (9.86 μm [5.77]). CTO caused the highest abrasive dentin wear (166.70 μm [69.90]), being statistically significantly higher than the wear for CP (54.20 μm [24.00]) and CWD (17.00 μm [7.80]) (p = 0.00001). The abrasive dentin wear for CWD was statistically significantly lower in comparison to all other groups (p = 0.00001).
Conclusion: Toothpastes with highly discrepant REA and RDA values presented statistically significantly different absolute wear on enamel and dentin. REA and RDA values should both be declared for every toothpaste.

Purpose: To evaluate the effects of acidic solutions on the surface roughness (SR) and surface microhardness (SMH) of indirect restorative materials by analyzing in vivo and in vitro studies.
Materials and Methods: Studies comparing the SR and SMH of indirect restorative materials after erosion were included in the metaanalysis. Meta-analyses were conducted with a random-effects model at a significance level of P < .05 using Review Manager software (Cochrane Collaboration). The literature search was conducted using the Web of Science, MEDLINE (PubMed), and Cochrane Library databases, with no limitations on publication year.
Results: Of the 1,097 potentially relevant studies, 32 full-text articles were assessed for eligibility. A total of 16 articles were included in the systematic review, and 15 were included in the meta-analyses. The kappa score between the two reviewers was 0.80. The effects of erosion on ceramics, indirect composites, and hybrid ceramics were evaluated by immersion in or rinsing with acidic solutions (eg, hydrochloric acid [HCl], cola drinks, and juices). An increase in the SR (Ra values) (P < .00001; mean difference [MD] = –0.04; 95% CI = –0.06 to –0.03) and a decrease in the SMH (Vickers microhardness [VHN]) (P < .00001; MD = 13.94; 95% CI = 12.33 to 15.55) were found after erosion. Subgroup analyses revealed that in vitro erosion led to significantly higher Ra values and lower VHN values than in vivo erosion; cola drinks and juices led to more surface degradation than HCl; and the SR and SMH of polymer-infiltrated ceramic network (PICN) materials remained unchanged after erosion.
Conclusion: After acidic challenges, significantly increased surface roughness and decreased surface microhardness were found on the surfaces of indirect restorative materials. Among all the indirect restorative materials, PICN showed the best acid resistance. Nevertheless, the changes in the SR and SMH of indirect restorative materials after erosion detected in the present study were minor and may not have any clinical significance.

Introduction: The restorative reconstruction of lost tooth structure and the restoration of the original vertical bite position in a dentition that has been severely affected by tooth erosion or abrasion always presents the dentist with problems in implementing the therapy. Traditionally, this therapy is carried out with laboratory-produced restaurations, which usually require preparation of the remaining tooth substance and result in high costs for the patient, so that alternative procedures should be considered. It should be emphasized that any restorative work, even minimally invasive, is only indicated in these patients if the preventive approach of inhibiting further loss of tooth structure is successful at the same time.
Treatment method: The following article presents a case study on the use of direct adhesive composite restorations as a possibility for reconstructing an erosively severely altered dentition. Transfer splints, which are fabricated on the basis of individual wax-up models, are used to reconstruct the occlusal surfaces.
Conclusion: The procedure described is a well-studied and proven method for restoring teeth with erosion-induced tooth wear. As with all new procedures, there will be a certain learning curve for the practicing dentist, after which high-quality restorations can be implemented using this technique.

Einführung: Der restaurative Aufbau verloren gegangener Zahnhartsubstanz und die Wiederherstellung der ursprünglichen vertikalen Bisslage bei einem durch Zahnerosionen oder -abrasionen stark in Mitleidenschaft gezogenen Gebiss stellt den Zahnarzt immer wieder vor Probleme bei der Umsetzung der Therapie. Traditionellerweise erfolgt diese Therapie mit laborgefertigten Werkstücken, die i.d.R. eine Präparation der verbliebenen Zahnsubstanz erfordern und für den Patienten hohe Kosten verursachen, sodass alternative Verfahren in Betracht gezogen werden sollten. Dabei gilt es hervorzuheben, dass jedwede, auch minimalinvasive restaurative Arbeiten bei diesen Patienten nur dann indiziert sind, wenn der präventive Ansatz, weiteren Zahnhartsubstanzverlust zu verhindern, gleichzeitig erfolgreich ist.
Behandlungsmethode: Der folgende Beitrag stellt die Verwendung direkter adhäsiver Kompositrestaurationen als eine Möglichkeit zur Rekonstruktion einer erosiv stark veränderten Gebisssituationen anhand eines Falls dar. Zur Rekonstruktion der Okklusalflächen werden dabei Übertragungsschienen eingesetzt, die auf der Basis von individuellen Wax-up-Modellen angefertigt werden.
Schlussfolgerung: Das beschriebene Verfahren ist eine gut untersuchte und erprobte Methode zur Versorgung von Zähnen mit erosionsbedingtem Zahnhartsubstanzverlust. Wie bei allen neuen Verfahren wird sich für den praktischen Zahnarzt eine gewisse Lernkurve ergeben, nach der hochwertige Restaurationen mit dieser Technik umgesetzt werden können.
