Mit einem Intraoralscanner wurden die Schleimhautoberflächen zahnloser Ober- und Unterkiefer sowie die Prothesenrandbereiche abgeformt. Zusätzlich wurden die vorhandenen Prothesen mit ihren Zahnpositionen und Prothesenbasen sowie ein mittels Pfeilwinkelaufzeichnung gewonnenes Zentrikregistrat mit dem Intraoralscanner digitalisiert. Diese Scans lieferten alle erforderlichen Informationen für die Neuversorgung mit CAD/CAM-gefertigten Totalprothesen. Ziel der geschilderten Untersuchung war, aktuelle Zahlen zu prothetischen Komplikationen bei Implantatprothesen, -brücken und -kronen zu ermitteln und mit den Zahlen einer Publikation aus dem Jahr 2003 zu vergleichen. Es zeigte sich, dass Implantat-Deckprothesen mit einer größeren Zahl von Komplikationsformen einhergehen als implantatgetragene Ganzkieferversorgungen, Implantatbrücken und Implantatkronen. Je nach Art der Prothetik ergaben sich unterschiedliche Komplikationen, beispielsweise für implantatgetragene Ganzkieferversorgungen Prothesenzahnfrakturen und für Implantatkronen Abutmentschraubenlockerungen. Neueste Abformtechniken setzen Intraoralscanner ein, um sowohl die Hartsubstanz als auch das Weichgewebe abzuformen. Allerdings bleibt zu untersuchen, ob Intraoralscanner über ausreichend Genauigkeit verfügen, um bei unbezahnten Patienten eine brauchbare Abformung für die Herstellung einer Totalprothese zu liefern. Ziel dieses Fallberichtes über die Behandlungen zweier Patienten ist es, eine Technik zu beschreiben, bei der ein Intraoralscanner eingesetzt wurde, um die Weichgewebemorphologie abzuformen und eine CAD/CAM-gefertigte Totalprothese herzustellen. Aim: To present recent data regarding prosthetic complications with implant prostheses and crowns as well as compare this data with data presented in a 2003 publication. Material and methods: An electronic Medline (PubMed) with MeSH terms search was performed, focussing on clinical studies that reported data on prosthetic complications associated with implant fixed complete dentures, implant overdentures, implant fixed partial dentures, and implant single crowns. Results: There were nine prosthetic complications reported with implant fixed complete dentures, 17 with implant overdentures, four with implant fixed partial dentures, and six with implant single crowns. The greatest number of complications and the largest incidence of percentages occurred with implant overdentures. The lowest incidence percentages were recorded for implant single crowns. These findings are in agreement with the previous 2003 publication. It is of interest to note that some of the complications reported previously were not reported in this review, and some complications reported in this review were not listed in the 2003 publication, thereby limiting the number of direct comparisons between this paper and the earlier report. A surprising finding was that some complications associated with implant overdentures from the current data exceeded the incidence in 2003 (reactivation of the retentive attachment; mucosal hyperplasia; and the need for overdenture relines). Conclusions: Implant overdentures are associated with more complications than implant fixed complete dentures, implant fixed partial dentures, and implant single crowns. The lowest incidence of complications was reported with implant single crowns. The most common complication reported with implant fixed complete dentures was denture tooth fracture. The most common complication associated with implant overdentures was the need for adjustments. Porcelain veneer fracture/chipping was the most common complication identified in the studies of implant fixed partial dentures. Intraoral scanning was used to capture the soft tissue surfaces of both maxillary and mandibular edentulous ridges and the denture borders. Additionally, an intraoral scanner was used to digitize existing dentures with their tooth positions and base forms and a centric relation record obtained with a Gothic arch-tracing device. These scans provided all the required records for fabrication of computer-aided design/computer-assisted manufacturing of complete dentures.

The newest impression techniques use intraoral scanners to capture both the hard and soft tissues. However, with edentulous patients, the accuracy of an intraoral scanner to capture an acceptable impression for the fabrication of a complete denture needs to be evaluated. Therefore, the purpose of this report of two patient treatments is to describe a technique that used intraoral scanning to record mucosal morphology and fabricate computer-aided design/computer-assisted manufacture (CAD/CAM) complete dentures.

Single implants and their crowns have high survival rates that exceed the survival rates for fixed partial dentures on teeth and most but not all publications have determined single implants are more cost-effective than 3-unit fixed partial dentures. Both initial root canal treatment and retreatment are more cost-effective than tooth extraction and rehabilitation with a single implant and crown. In diesem zweiten Teil der Kasuistik werden die technischen Schritte zur Herstellung der digital geplanten Unterkiefertotalprothese beschrieben. Diese wird mit einem eingearbeiteten, gefrästen Titansteg befestigt; das Gegenstück im Oberkiefer ist eine CAD/CAM-gefertigte definitive Oberkiefertotalprothese. Eine digital hergestellte Implantatregistrierungshilfe dient als Abformung und Grundlage für die definitive Unterkieferprothese. Sie funktioniert nicht nur als Abformlöffel, sie ermöglicht auch eine Bissregistrierung in korrekter okklusaler Dimension. Dieser Artikel beschreibt ein spezifisches prothetisch gesteuertes Protokoll, um mithilfe von computergeführter technischer Planung einen hochentwickelten, wissenschaftlichen Algorithmus zu entwickeln, der den Herstellungsprozess zur Umwandlung einer Prothese mithilfe des AvaDent Digital Denture Systems (Global Dental Science) steuert. Dieses System ist mit der NobelClinician (Nobel Biocare) Implantatplanungssoftware kombiniert, um die Präzision zu optimieren und zur leichteren und schnelleren Umwandlung einer Prothese in eine sofortbelastbare, implantatgetragene, provisorische, festsitzende Totalprothese unmittelbar nach dem Einbringen der Implantate, unter Zuhilfenahme einer NobelGuide chirurgischen Bohrschablone.
