Guided Surgery For Advanced Users – Treatment Plan as the Key To Success Dr. Tali Chackartchi, Prof. Dr. Ignacio Sanz Sànchez
Dental implants are highly predictable in terms of implant survival. However, today we need to
consider many critical factors to achieve long-term success. One of the most important ones is
treatment planning. We need to consider that dental implantology is a prosthetic discipline with a
surgical component and, therefore, it is mandatory to merge the radiographic information with the wax
up to plan the implants and the surgery based on the designed future restoration.
With the advent of new technologies and the demand from the patient for immediacy, immediate
loading protocols have become very popular and routine treatment in the clinic. To make this process
much easier and predictable, the combination of a surgical guide with a prosthetic guide may be
consider. This protocol is based on using common anchoring pins on different guides, giving the
chance to place the implants completely guided and then to place the provisional restoration with a
second guide.
In the present workshop, we will review the key factors when planning digitally an implant case. We
will evaluate which elements we need to take into consideration to improve the accuracy of guided
surgery, and we will present several clinical cases using the prosthetic guide approach.
Moreover, we will run a hands-on workshop with the following

learning objectives:

* To evaluate prosthetically-driven planning for immediate loading procedures on a partial edentulous case.
* To combine a surgical MGUIDE and a prosthetic MGUIDE for a partial bridge case.
* To review the tricks for the guided surgery of the conical implant system.
* To do an immediate loading procedure using the prosthetic MGUIDE after placing the implants fully guided.

Soft Tissue Harvesting Techniques Prof. Dr. Moshe Goldstein
Soft Tissue grafting is an important part of modern Implant Dentistry. Soft tissue grafts are used to
increase the volume of Keratinized Tissues before, during, or after implant placement. They are
imperative for Vestibulopathy procedures. Have a major role in the management of extraction sites
before or concomitant with implant placement, especially in the esthetic zone, and significant
contribution when aiming to repair soft tissue deficiencies at functioning implants.
The main donor site for autologous soft tissue is the palate and there are several techniques of soft
tissue harvesting. The aim of this course is to familiarize the participants with the different soft tissue
harvesting techniques. Their specifics, advantages and disadvantages.

Management of Cases with High Aesthetic Demand, the Position of the Tissue and the Implant
Aesthetics has become one of the main motivations of patients at dental offices today. The oro-facial harmony does not depend only on the position, color, shape, or size of the teeth but also on its direct relationship with the position and architecture of the tissue, which can determine the failure or success of the case. The concept of “TISSUE” implies aesthetics and function. The different mucogingival techniques are designed to correct the position, volume and characteristics of the tissue, to obtain a perfect end for our cases around teeth and implants. Digital dentistry gives us the possibility to plan where the tissue and implant position should be before we start with the case; this way, we will only need to follow the steps of the planification and so obtain a predictable and perfect end. Technical Aspects in the Surgical Treatment of Peri-Implantitis Alberto Monje
Peri-implantitis is a pathogenic inflammatory condition characterized by progressive bone loss and clinical inflammation that may compromise the stability of dental implants. Therapeutic modalities have been advocated to arrest the disorder and to establish peri-implant health. Reconstructive therapy is indicated for bone defects exhibiting contained/angular components.
This therapeutic modality is based on the application of the biological and technical principles of periodontal regeneration. On the other hand, respective interventions aim at closing the pocket by means of osteoplasty and/or mucosal resection. It is yet unclear the step-by-step approach to achieve success conveying the potential and limitations. Therefore, the aim of this lecture is to provide the technical foundation in the surgical therapy of peri-implantitis.

Avoiding Complications in Vertical GBR Procedures Dr. Isabella Rocchietta
Dental implants have been successfully implemented as a routine therapeutic approach in partially and fully edentulous patients. Alveolar defects compromise the “prosthetically-driven” dental implant positioning, unless detailed planning is performed prior to the surgical phase. Many techniques have been described to augment the alveolar bone, including autogenous block grafts, titanium mesh, distraction osteogenesis, and guided bone regeneration, just to name a few.
The use of Guided Bone Regeneration associated with non-autogenous scaffolds constitutes a solid therapy for bone augmentation in the vast majority of alveolar defects with or without implants. Choosing the correct therapeutic approach with the appropriate biomaterials as well as soft tissue management are fundamental players to achieve success in advanced GBR procedures. However, complications are the biggest threat that jeopardizes the final result. Prevention is crucial to avoid complications, but when they do occur, the delicate management changes the course of events, often resulting in a successful outcome. A decision tree based on clinical experience and supported by the literature is important to master and be prepared for any adverse event that may occur prior to, during, or after a complex surgery such as GBR.

Peri-Implantitis Prevention: Understanding Local-and Patient-Related Risk Factors Dr. med. dent. Aušra Ramanauskaitė
Peri-implantitis is a plaque-associated pathological condition characterized by mucosal inflammation along with progressive loss of supporting bone. Substantial evidence suggests that non-surgical therapy is of limited efficacy in arresting further bone loss, thus indicating the necessity of surgical intervention in the majority of patients diagnosed with peri-implantitis.
Achieving optimal treatment results in diverse clinical situations requires proper knowledge of indications and surgical performance of different surgical treatment modalities. This lecture will cover different surgical peri–implantitis treatment concepts, including reconstructive, resective, and combined surgical therapies, and the decisive factors in determining which surgical technique is most appropriate in different peri-implantitis cases. Important aspects of soft-tissue management and the rationale for soft-tissue volume grafting following surgical peri-implantitis treatment, as well as the prognostic factors associated with treatment outcomes, will be further discussed.

Future Trends of Bone Regeneration, Tissue Engineering and Cell Therapy Prof Mariano Sanz
Current bone regenerative interventions are based on tissue engineering principles, using different combinations of:

a. cells with the ability to differentiate into bone-producing cells (mature osteoblasts);
b. bone replacement grafts as scaffolds providing not only a space maintenance effect, but also intrinsic properties to guide or induce new bone formation (osteoconduction or osteoinduction);
c. signaling molecules that guide the wound healing process towards new bone formation and full defect regeneration. In this presentation, we shall focus on one of the principles, the effective use of cells, by describing the scientific background and the clinical application of autologous stromal cells harvested from the iliac crest bone marrow and characterized and expanded ex vivo.

We shall present the results from a recent clinical trial on mandibular vertical bone regeneration, and we shall elaborate on future perspectives of this technology and on the use of cellproducts (secretome) as signaling molecules for bone regeneration.

Placements of implants and immediate loading of a full arch is an advanced clinical procedure that requires meticulous planning and a team approach that has to be perfectly coordinated.
Furthermore, traditional static navigation has some limitations even when bone-supported surgical guides are used in edentulous or partially edentulous patients with terminal dentition.
Subsequently, the exchange of the surgical guide to the prosthetic guide further complicates the accurate positioning and the final delivery of the provisional prosthesis.
The aim of this session is to provide some innovative solutions to enhance the accuracy of such procedures using the MGUIDE system.

Natural Mucosal Contour around Implants: Techniques, Execution and Critical Decision-Making Dr. Serhat Aslan
When natural dentition is combined with dental implants, treatment modalities become more complex due to anatomical differences. Blood supply, handling of the soft tissues, flap thickness, design and tension are key elements to achieve successful surgical outcome. These factors should be evaluated and controlled during the surgical intervention. Creating the Most Personalized Smile Design and a Guided Approach to Reach It: Analog vs Digital Dr. Galip Gürel, CDT Hilal Kuday, Dr. Stefen Koubi
While some of the final esthetic results are creating the “wow” effect, some may fail to meet the patient's expectations due to disharmony between the smile design and the patient's personality. The patient may feel that the restored teeth do not really “belong” to him or her. Without the proper knowledge, the origin of this disharmony can be difficult to identify. Most of all the smiles are being designed intuitively by the dentist or the ceramist. It has always been a challenge to create a smile design so that the final result would be precisely predicted from day one. In some cases, the dentists build this design, in other words, “the mock-up,” by themselves, directly in the patient's mouth. However, some leave it to the ceramist that they
work with. More importantly, this smile design does not always match the patient’s functional, biological and emotional needs. This brings out the extreme need and precision of the “mock-up.”
The aim of this lecture is to present a novel concept that involves the creation of a mock-up that will be matched with a customized personal image, expressing a person's sense of identity which is based on the “Visagism” VIS concept that helps dental clinicians provide restorations that account not only for esthetics, but also for the psychosocial features of the created image, which affect patients’ emotions, sense of identity, behavior, and self-esteem. Even with this very valuable VIS smile design, for some dentists, creating a direct mock-up may be difficult. Filling an empty space with teeth that will fulfill all the above expectations can be a difficult task. However, with the new digital technologies, this is no longer a problem.
The REBEL is the new 3D SMILE DESIGN program, which can immediately convert the 2D VIS smile design into 3D. This way, the dentist will be able to immediately 3D print this STL file and be able to place the final wax up in the patient's mouth. Of course, if the design is the crucial point, a guided clinical protocol where all the steps are standardized is mandatory.
The challenge of the esthetic zone should always follow a predictable protocol because there is no room for failure in this crucial area. Whatever the complexity of the clinical situation (Worn dentition, implantology), the practitioner needs to use a comprehensive methodology with a simple philosophy able to guide the treatment during the different clinical steps. The purpose of this session is to suggest a precise and reproducible method that simplifies the treatment of smile enhancement through implantology. A wax-up and a mock-up are mostly used for designing aesthetic outcomes and driving implant position accordingly. Darko Božić is an Associate prof at the Department of Periodontology, School of Dental Medicine, Zagreb, Croatia. He works at the University Clinical Hospital at the Clinic for periodontology as a specialist in periodontology. In addition to that, he also works every day in his private clinic. Currently he holds the position of the President of the Croatian Society of Periodontology, and is a member of the EFP executive committee and will be the president of the EFP in 2023-24. The focus of his research is proteomic analysis of periodontal tissues for which he and his team received two prestigious IADR awards. His main focus in his clinical work is the reconstruction of soft and hard tissues in periodontitis affected patients with a special research interest is the application of growth factors, namely BMPs and the Hyaluronic acid in regenerative procedures as well as soft tissue reconstruction utilizing various collagen matrices. He lectures extensively on the national and international level on various topics with more than 70 invited lectures so far, many at international periodontal congresses. Professor Goldstein is Chairman of the Israel Periodontal Society and President of the European Federation of Periodontology. He is a lecturer of worldwide authority and a clinical specialist. Dr. Stefen Koubi

Doctor Stefen Koubi graduated from the University of Marseille, France, in 1998. He has held the position of Associate Professor in the Restorative Department of Marseille University since 2008 and received his PhD in 2011. He maintains a private practice in Marseille and in Paris. His main areas of interest are esthetic rehabilitation for cosmetic or functional reasons (worn dentition). He has been invited to many prestigious esthetic academies to lecture on simple ways to achieve high-quality esthetic dentistry. He is an invited professor in many esthetic postgraduate programs. Dr Koubi received the Gold Molar title from the students of his university (best teacher of the year) in 2012, and the title of Best French Speaker the same year. Dr. Stavros Pelekanos received his undergraduate degree in Dentistry (D.D.S.) from the Univ. of Athens, Greece. In 1993, he obtained his doctoral degree in Prosthodontics (DMD) from the Univ. of Freiburg, Germany. Ariel J. Raigrodski DMD, MS

Ariel J. Raigrodski, DMD, MS, received his DMD degree from the Hebrew University in Jerusalem, Israel. He went on to receive his certificate in prosthodontics from Louisiane State University School of Dentistry, where he also completed a fellowship in implants and esthetics and an MS degree. Dr Raigrodski is currently a professor in the Department of Restorative Dentistry and an adjunct professor in the Department of Washington. He is a Diplomate of the American Board of Prosthodontics, a Fellow of the American College of Prosthodontists and the International College of Dentists, a member of many professional organizations, and a member of several editorial boards for professional journals. Dr Raigrodski's research focuses on ceramics, implant esthetics, and CAD/CAM technology. He has published extensively and lectures both nationally and internationally. Dr Raigrodski maintains a private practice limited to restorative, implant, and esthetic dentistry in Kenmore, Washington.

Dr. med. dent. Aušra Ramanauskaitė PhD, MSc, DDS

Ausra Ramanauskaite completed the dental education (DDS) (2006–2011) and specialization in periodontology (MSc) (2011–2014) at the Lithuanian University of Health Sciences, Lithuania. In 2016, she was awarded a one-year Osteology Foundation research scholarship at the Oral Surgery Department, University of Düsseldorf, Germany. Isabella Rocchietta, DDS, MSc, is an honorary senior research associate to the Department of Periodontology and a lecturer for the Diploma in Implant Dentistry at the UCL Eastman Dental Institute in London. Prof Mariano Sanz MD

Mariano Sanz is professor and chair of periodontology at the University Complutense of Madrid (Spain) and a professor in the faculty of Odontology at the University of Oslo (Norway). He graduated in medicine in 1981 from the University Complutense of Madrid, from which he received a degree in stomatology in 1983 and where he qualified as a Doctor of Medicine in 1985. He received his speciality in periodontology from the Universtiy of Califormia in Los Angeles (UCLA) in 1987. He has received honorary doctorates from the University of San Sebastian in Santiago (Chile), the University of Gothenburg (Sweden) and the University of Coimbra (Portugal). Since 2005, Prof Sanz has been chair of the ETEP research group on the aetiology and therapy of periodontal diseases, whose main lines of research are oral microbiology, bacterial-host interactions, and antimicrobial approaches in the treatment of gingivitis and periodontitis. The research group has conducted clinical trials to measure the efficacy of different approaches to periodontal regeneration surgical protocols using dental implants, and therapeutic approaches to treat peri-implantitis. Prof Sanz has published 230 articles in scientific journals, written 50 book chapters, and has participated as an invited speaker at more than 200 scientific events in the last five years. He is an associate editor of the Journal of Clinical Periodontology and Evidence-Based Dental Practice and a member of the editorial boards of various other dental journals. He has been awarded the Jens Waerhaug Research Prize by the Scandinavian Society of Periodontology (1984), the Outstanding Service Award from the International Association for Dental Research (2015) and the IADR Straumann Award in Periodontal Regenerative Medicine (2015). He is a member of the executive committee of the European Federation of Periodontology (EFP) and chair of its workshop committee. He has previously served the EFP both as president (1993-1994) and secretary general (1998–2005). He is president of the Osteology Foundation, president of the International Association for Dental Research’s continental European division, and president-elect of the Association for Dental Education in Europe (ADEE).
