Nitzan Bichacho, D.M.D. holds the post of expert in prosthodontics at the rank of Professor, at the Faculty of Dental Medicine, Hebrew University and Hadassah, Jerusalem, Israel, where he graduated in 1984. Purpose: To describe a digital workflow for the management of the emergence profile and the restoration’s finishing line on vertically prepared teeth using a digital data set. Materials and Methods: Three scans (the prepared teeth the day of the preparation, the provisional restoration placed on the preparation after soft tissue maturation, and the provisional restoration outside of the oral cavity) were combined under a common data set to provide the necessary information to design the final restoration. Results: The digital technique presented can facilitate the implementation of the vertical preparation, because the desired finish line can be defined by the clinician and be transferred predictably to the dental lab. The current approach provides an alternative gingival management method, taking advantage of the displacement of soft tissue after vertical preparation. Conclusions: Positioning of the final finishing line on vertically prepared teeth, management of soft tissue, and fabrication of the final restoration can be facilitated with this approach, combining a digital data set that has been obtained during a distinct phase of the treatment.
The clinician's selection of an implant system is influenced by many variables. Ideally, the decision should be based on scientific evidence, but often these decisions are based on economic considerations or influenced by the experience of a trusted peer. The purpose of this paper is to describe the influence of implant neck features (shape and surface) and abutment connection (diameter that matches or is smaller than the implant's platform) on hard and soft tissues around single-tooth implants placed into healed ridges with adequate hard and soft tissue thickness. In an effort to reduce the number of variables, only two-piece implants fully placed at bone level or beneath were taken into consideration. The goal is to provide additional guidance for clinicians on the decision-making and implant-selection processes.
Die ästhetische Rehabilitation der Patienten verlangt nach vereinfachten Behandlungsabläufen mit vorhersagbaren Ergebnissen. Die vorgestellte Kasuistik zeigt eine solche Technik zur einfachen, sicheren Augmentation und Entwicklung eines weichgewebigen Pontic-Auflagers mit Bindegewebstransplantation. Nach einer präzisen Abformung wird die Augmentation zunächst auf dem Modell "realistisch" simuliert und das definitive Pontic entwickelt. Unmittelbar nach der chirurgischen Augmentation eingebracht, formt es das heilende Weichgewebe dann ideal aus.

Restaurativer Ansatz unter besonderer Berücksichtigung von Gingivaintegration und Papillenunterstützung
Viele Patienten lehnen bei Zahnfehlstellungen kieferorthopädische Behandlungen ab. In manchen dieser Fälle kann eine rein restaurative Behandlungsstrategie zwar keine optimalen, aber immerhin zufriedenstellende Ergebnisse liefern. Der Beitrag erörtert diverse ästhetische Probleme im Zusammenhang mit Zahnfehlstellungen. Alle Fälle sind dadurch gekennzeichnet, dass lediglich eine indirekte restaurative Behandlung gewählt wurde. Das Ziel bestand jeweils in zufriedenstellenden Resultaten, die aber nicht ganz perfekt sein mussten. Die Behandlungen sind jeweils als annehmbarer Kompromiss zu betrachten, der innerhalb der vom Befund gesetzten Grenze eingegangen wurde.

Modular Transitional Implants (MTI) are made from pure titanium and are used to support fixed provisional restorations during the osseointegration of definitive implants. This study histologically examined the jaw response to loaded MTIs in the dog mandible. Three implants were inserted transmucosally into each side of the mandible in 3 dogs. Stability was examined using a Periotest. Anterior and posterior implants were splinted using a cemented acrylic resin fixed partial denture to allow immediate loading. The middle implant remained unloaded and was used as a control. Dogs were sacrificed 11 to 12 weeks after implantation, and tissue blocks containing the implants were removed. Histologic examination showed that 10 of the 18 implants had good bone-to-implant contact, with the percentage of bone contacting the threaded portion of the implant varying from 30% to 65%. There was no statistical difference (P > 0.1) in percentage of bone-to-metal contact between loaded and unloaded implants. Six implants were entirely surrounded by connective tissue with or without inflammation; two implants were lost during the study. The success rate did not differ between loaded and unloaded implants. In the successful implants trabecular bone made good contact with the implant, forming supporting struts.There was bone remodeling in some bone-to-metal contact areas. It is believed that success was mainly influenced by the initial bone density at the implant site and by the uncontrolled load that the animals applied to the implants during the early healing stage.
