Es ist allgemein akzeptiert, dass der Erfolg von implantatgetragenen Versorgungen nicht nur auf der Osseointegration beruht, sondern auch darauf, ein ästhetisches Erscheinungsbild und die Gesundheit der Gewebe zu erreichen. Eine harmonisch aussehende "pink Ästhetik" zu schaffen, gehört zu den größten Herausforderungen bei der implantatgetragenen Versorgung im Frontbereich. Dies gilt insbesondere für zwei nebeneinanderliegende Implantate. Zwei Komponenten beeinflussen die Gingiva um Implantate: eine adäquate Unterstützung durch den Knochen und eine ausreichende Quantität und Qualität der Weichgewebe. In verschiedenen Artikeln wurde auf die Notwendigkeit der Regeneration und des Knochenerhalts nach der Extraktion oder der Freilegung der Implantate hingewiesen. Bei klassischen Implantatprotokollen sind immer mehrere Eingriffe im Implantatbereich notwendig sowie die chirurgische Manipulation der periimplantären Gewebe. Durch den vorsichtigen chirurgischen Umgang mit den Weichgeweben bei der Freilegung der Implantate und die Eingliederung der Heilungskappen (Zweiteingriff) kann der Behandler das bestmögliche Ergebnis erreichen. Dennoch besteht das Risiko, dass durch diesen Eingriff eine Schwächung und Vernarbung der Gewebe entsteht. In der vorliegenden Studie wird ein neues Protokoll vorgeschlagen, das den Einsatz eines Bindegewebstransplantats und die Remodellierung der Weichgewebe vorsieht, die auf der Ausformung ovoider Pontics beruht. Mithilfe dieser Technik kann die Narbenbildung nach einem Zweiteingriff reduziert und das endgültige Austrittsprofil verbessert werden.

It is currently accepted that success in implant-supported restorations is based not only on osseointegration, but also on achieving the esthetic outcome of natural teeth and healthy soft tissues. The socalled "pink esthetic" has become the main challenge with implant-supported rehabilitations in the anterior area. This is especially difficult in the cases with two adjacent implants. Two components affect the final periimplant gingiva: a correct bone support, and a sufficient quantity and quality of soft tissues. Several papers have emphasized the need to regenerate and preserve the bone after extractions, or after the exposure of the implants to the oral environment. The classical implantation protocol entails entering the working area several times and always involves the surgical manipulation of peri-implant tissues. Careful surgical handling of the soft tissues when exposing the implants and placing the healing abutments (second surgery) helps the clinician to obtain the best possible results, but even so there is a loss of volume of the tissues as they become weaker and more rigid after each procedure. The present study proposes a new protocol that includes the connective tissue graft placement and the soft tissues remodeling technique, which is based on the use of the ovoid pontics. This technique may help to minimize the logical scar reaction after the second surgery and to improve the final emergence profile.

The use of magnetic resonance imaging as an adjunctive aid to determine the amount of bone available and the anatomic limitations for the placement of endosseous dental implants is introduced. Advantages and disadvantages are discussed. The results suggest that magnetic resonance imaging can provide, without the need for a reformatting software program, adequate information concerning the amount of bone and anatomic limitations for placement of endosseous implants in the mandible and maxilla.
