Die Prävention der Primärkaries ist in der Bundesrepublik Deutschland sehr erfolgreich, 50 % weniger direkte Restaurationen in 30 Jahren sind ein großer Erfolg. Zugleich hat ein Paradigmenwechsel weg von den klassischen Black-Präparationsregeln und hin zu substanzschonendem und minimalinvasivem Vorgehen stattgefunden. Es gibt vor dem Hintergrund des Amalgamverbots aktuell mehrere neue und vielversprechende selbstadhäsive Materialien und Materialgruppen auf dem Markt. Die klinische Datenlage ist allerdings extrem unterschiedlich. Die Indikation für direkte Seitenzahnrestaurationen hängt heute nicht nur von der Kavitätengröße ab, sondern von zahlreichen anderen Faktoren wie Compliance (vulnerable Gruppen), Kariesrisiko, Parafunktionen/Bruxismus etc. Das bedeutet, dass nicht ein Material Amalgam ersetzen wird, sondern dass der Einsatz von Alternativmaterialien indikationsbezogen erfolgen muss. In bestimmten Indikationsbereichen sind hochvisköse Glasionomerzemente (GIZ), GIZ-Komposit Hybrid-Materialien oder Alkasite gut einsetzbar und haben das Potenzial für eine Basisversorgung, wobei im Einzelfall und je nach Einsatzgebiet variierende Überlebenszeiten auftreten können und werden. Deshalb muss der Zahnarzt bzw. die Zahnärztin in Absprache mit den Patientinnen und Patienten individuell entscheiden, welches Material in der jeweiligen Indikation infrage kommt. Amalgam in der Zahnmedizin ist demnächst nicht mehr verfügbar. Dieser Wechsel könnte die langjährige Diskussion um die Biokompatibilität beenden, wirft aber auch neue Fragen auf: Wird dadurch wirklich alles besser? Im Folgenden werden die verfügbaren Alternativen zu Amalgam hinsichtlich ihrer Biokompatibilität und möglicher gesundheitlicher Folgen durch freigesetzte Inhaltsstoffe diskutiert. Anhand von 10 Thesen soll aufgezeigt werden, dass die Biokompatibilität von Füllungsmaterialien auch in Zukunft von großer klinischer Bedeutung sein wird und dass – bedingt durch die sehr heterogene Zusammensetzung vieler dieser Materialien – nicht zwingend alles besser, sondern eher komplexer wird. Purpose: This follow-up of a randomized clinical split-mouth study aimed to investigate the influence of selective enamel etching on the long-term clinical performance of partial ceramic crowns (PCCs) luted with a self-adhesive resin cement. Materials and Methods: 43 patients received two PCCs (Vita Mark II; Cerec 3D) each for the restoration of extensive lesions with multiple cusp coverage, inserted with a self-adhesive resin cement (RelyX Unicem, RXU). Using a split-mouth design, one PCC received additional selective enamel etching (RXU+E) and one did not (RXU-E). Patients were clinically evaluated at baseline and after up to 15 years (median observation period 176 months) using modified USPHS and FDI criteria. The data were analyzed non-parametrically (chi-squared tests, α = 0.05). Clinical survival of all restorations after 15 years was evaluated using the Kaplan-Meier analysis. Results: After 15 years, 19 patients were available for clinical assessment (recall rate: 56%). Kaplan-Meier analysis showed a cumulative survival of 78.1% for RXU+E and of 42.9% for RXU-E, indicating a significantly higher survival rate for RXU+E (p = 0.004). Regarding the clinical performance of PCCs available for the 15-year evaluation, no statistically significant differences were found between RXU+E and RXU-E using modified USPHS and FDI criteria. Both groups revealed significant deterioration over time regarding surface luster, marginal adaptation, and marginal discoloration. RXU+E resulted in significantly inferior anatomic form over time and a significant improvement in post-operative hypersensitivity compared to baseline. Conclusion: For posterior PCCs, selective enamel etching can be recommended based on higher survival rates after 15 years. Clinically, deterioration due to aging is similar in both groups. Summary: The Minamata Convention resulted in restrictions in the use of amalgam in daily dental practice. This opens up new discussions about the biocompatibility of amalgam, but also of composites as alternative materials. In the following review article, these issues will be discussed in more detail to provide dentists with a knowledge base for themselves and for communication with their patients. In addition to mercury in amalgam or monomers in composites, bisphenol A and nanoparticles generated during the grinding, polishing or removal of restorations must also be included in the biocompatibility evaluation. In laboratory tests, these substances cause toxic reactions, and bisphenol A also exhibits estrogen-like effects. However, it must be taken into account that the concentrations used in laboratory tests are much higher than in clinical practice. Thus, both amalgam and composite can be used in the general population. Nevertheless, for scientifically, politically and legally defined risk groups (e.g. dental personnel, allergic persons, pregnant or lactating women, children under 15 years of age, people with certain systemic diseases), indication restrictions and precautionary measures must be observed. In Laborversuchen rufen diese Substanzen toxische Reaktionen hervor und Bisphenol A zeigt zudem eine östrogenartige Wirkung. Allerdings sind die applizierten Konzentrationen in Laboruntersuchungen wesentlich höher als im klinischen Alltag. Deshalb können sowohl Amalgam als auch Komposit bei der Allgemeinbevölkerung eingesetzt werden. Hingegen sind bei wissenschaftlich und politisch definierten Risikogruppen (zahnärztliches Personal, Allergiker, Schwangere bzw. Stillende, Kinder unter 15 Jahren, Menschen mit speziellen Allgemeinerkrankungen) Indikationseinschränkungen und Vorsichtsmaßnahmen zu beachten. Die bekannte Amalgamdiskussion hat uns dabei die Bedeutung einer eingehenden und offenen Risikokommunikation mit dem Patienten gelehrt.

Purpose: To investigate the influence of selective enamel etching on long-term clinical performance of partial ceramic crowns (PCCs) luted with a self-adhesive luting material (RXU: RelyX Unicem). Materials and Methods: At baseline, 34 patients received the intended treatment: two PCCs (Vita Mark II; Cerec 3D) for the restoration of extended lesions with multiple-cusp coverage were placed in a split-mouth design with a self-adhesive luting material, one without (RXU) and one with selective enamel etching (RXU+E). Patients were evaluated clinically (modified USPHS criteria) at baseline and up to 6.5 years (70 to 88 months). The chi-square test was used for statistical analyses (α = 0.05). Clinical survival of all restorations (n = 68) after 6.5 years was evaluated by Kaplan-Meier analysis. Results: After 6.5 years, 18 patients (9 male, 9 female; median age 41, range 25 to 59 years) with 36 RXU and RXU+E restorations were available for clinical assessment (patient recall rate: 53%), with 13 RXU and 14 RXU+E PCCs placed in molars and 5 RXU and 4 RXU+E PCCs in premolars. Clinically, no statistically significant differences between the luting procedures were detected. Both RXU and RXU+E revealed significant changes over time with respect to marginal adaptation (significant deterioration) and marginal discoloration (significant increase). RXU revealed no cases of postoperative hypersensitivity and RXU+E only did so at baseline (n = 5). Kaplan-Meier analysis showed a cumulative survival for RXU of 60% and for RXU+E of 82%, indicating a significantly higher survival rate for RXU+E. Conclusion: Clinically, RXU and RXU+E perform similarly. In PCC restorations with multiple-cusp coverage, lack of retention due to adhesive preparation, and little dentin available for adhesion caused by extensive core buildups or cavity linings, selective enamel etching is recommended. Dental stem cells are excellent for oral and craniofacial tissue engineering. A profound knowledge about molecular processes in dental stem cells is necessary to create treatment approaches in oral medicine. Transcription factors regulate gene expression and provide decisive information for cellular functions. In recent years, the authors have investigated transcriptomes in dental stem cells before and after osteogenic differentiation. The present paper reports on the potential role of selected transcription factors, including ZBTB16, TP53, and SP1, in dental stem cell differentiation. This review discusses putative molecular processes in dental stem cells and summarizes the current knowledge.
