To improve the adaptation to bone level discrepancies of sloped alveolar crests, an implant with a sloped implant-abutment interface has been developed. In a previously reported international multicenter study, 65 sloped implants replaced single teeth with a buccolingual bone level discrepancy at least 3 months after tooth loss. The present study reports the long-term outcomes of one study center and also evaluates marginal bone and keratinized mucosa alterations around sloped implants in healed, sloped crests in a long-term follow-up. Implant survival, marginal bone levels, and buccal bone dimensions in relation to the implant shoulder level (assessed by periapical radiographs and CBCT), width of keratinized mucosa, and probing depths were observed. Two patients did not present for follow-up examinations. No implants were lost. The remaining 13 implants were followed for a mean period of 123 months. Mean marginal bone levels were maintained slightly coronal to implant shoulder level at the interproximal (0.13 mm) and the buccal aspects (0.57 mm). The width of keratinized mucosa improved significantly from delivery (1.85 mm) to the 1-year follow-up (3.39 mm) and was thereafter unchanged. Placing implants with a sloped shoulder in a healed crest with lingual-buccal bone level discrepancy resulted in circumferential marginal bone maintenance and improved width of the keratinized mucosa.

Ziel der prospektiven Studie ist es, das Überleben und die periimplantäre Hart- und Weichgewebestabilität nach Sofortimplantation von von Astra Tech OsseoSpeed EV 5.4S Implantataten in Molarenextraktionsalveolen und periimplantärer Defektaugmentation mit autologen Knochenspänen oder alloplastischem Knochenersatzmaterial (KEM) zu evaluieren. Material und Methoden: 50 Implantate wurden inseriert. Die periimplantäre Defektaugmentation erfolgte randomisiert mit autologem Knochen (AK) oder einem biphasischem KEM (Symbios, SY). Primäres Erfolgskriterium war das Implantatüberleben, als sekundäre Kriterien wurden das marginale Knochenniveau, die orobukkale Alveolarkammbreite, die periimplantären Sondierungstiefen und der Implantaterfolg beurteilt. Ergebnisse: Ein Implantat der SY-Gruppe ging verloren. 49 Implantate wurden durchschnittlich 14 Monate nachuntersucht. Die Überlebenswahrscheinlichkeit betrug bei der AK-Gruppe 100 %, bei der SY-Gruppe 96 %. Das Knochenniveau regenerierte von −7,5 mm bei Insertion auf −0,3 mm unterhalb der Implantatschulter bei der letzten Nachuntersuchung. Die Alveolarkammbreite änderte sich um −0,9 mm (AK) bzw. +0,3 mm (SY) 1 mm apikal der Implantatschulter, um −1,1 mm (AK) bzw. +0,2 mm (SY) 3 mm und um −0,8 mm (AK) bzw. +0,6 mm (SY) 6 mm apikal des Niveaus. Acht Implantate (3 AK, 5 SY) zeigten einen approximalen Knochenverlust > 1 mm oder Sondierungstiefen > 5 mm. Zusammenfassung: Die ersten Ergebnisse belegen eine hohe Überlebenswahrscheinlichkeit, eine gute Defektregeneration und eine geringe orobukkale Resorption bei Molarensofortimplantaten ohne signifikante Unterschiede für die Augmentationsmaterialien. Ein voluminöseres Implantatdesign könnte die Kongruenz zwischen einer Molarenextraktionsalveole und einem Zahnimplantat verbessern und neben einer verbesserten Primärstabilität auch zu einer geringeren orobukkalen Knochenresorption führen. Ziel dieser retrospektiven, monozentrischen Kohortenstudie war daher, die klinische Anwendbarkeit von Implantanten des Typs Astra Tech Implant System (ATIS) OsseoSpeed™ EV 5,4 S als Sofortimplantat in Molarenextraktionsalveolen in einem Nachuntersuchungszeitraum von ein bis vier Jahren zu prüfen. Material und Methoden: Bei 58 Patienten wurden 65 Implantate als Sofortimplantat in das Zentrum einer Molarenextraktionsalveole inseriert. Die periimplantären Defekte wurden simultan mit autologen Knochenspänen vom Unterkieferwinkel augmentiert. Alle Implantate wurden mit Healingabutments für eine transgingivale offene Einheilung versorgt. Nach drei Monaten Einheilung erfolgte die definitive Versorgung. Primäres Erfolgskriterium war das Implantatüberleben, als sekundäre Kriterien wurden das marginale Knochenniveau, die orobukkale Breite des Alveolarkamms, die periimplantären Sondierungstiefen und der Implantaterfolg beurteilt. Ergebnisse: Fünf Patienten mit 6 Implantaten erschienen nicht zur Implantatkontrolle (drop-out). Die verbliebenen 59 Implantate bei 53 Patienten (28 Männer, 25 Frauen, Durchschnittsalter 53,5 Jahre) wurden durchschnittlich 24 Monate nachuntersucht. Kein Implantat ging verloren. Das approximale Knochenniveau regenerierte von −6,6 mm unterhalb des Implantatschulterniveaus bis auf das Schulterniveau bei der letzten Nachuntersuchung. Die orobukkale Breite des Alveolarkamms veränderte sich von 10,6 mm auf 9,8 mm (-7,5%) auf dem Niveau 1 mm apikal der Implantatschulter, von 11,2 mm auf 10,9 mm (2,7 %) auf 3 mm und von 12,3 mm auf 11,8 mm (-4,1 %) auf 6 mm. Vier Implantate zeigten einen approximalen Knochenverlust von mehr als 1 mm unterhalb des Schulterniveaus oder Sondierungstiefen von mehr als 5 mm. Rauchen, unzureichende Mundhygiene und parodontale Vorerkrankung erhöhten das Risiko für die Implantatversorgung. Zusammenfassung: Die klinischen und radiologischen Ergebnisse belegen, dass ein breiteres Implantatdesign den marginal Knochen zirkulär erhalten kann und in der Lage ist, das Ausmaß der orobukkalen Resorption zu reduzieren, wenn es als Sofortimplantat in Molarenextraktionsalveolen inseriert wird. Background: A stable marginal bone and healthy peri-implant mucosa define the fundament for long-term stability of dental implants and both have impact on a satisfying esthetical and functional outcome. Apart from surgery, implant design and prosthetic parts have potential influence on these important outcome parameters. The CONELOG® SCREW-LINE implant by CAMLOG Biotechnologies AG offers a promising implant design with conical abutment connection and integrated platform shift. Aim: Evaluation of radiographic bone level changes from time of implant placement up to 60 months after prosthetic delivery by intraoral standardized x-rays in a prospective observational study. Secondary parameters were survival rate, performance of restorative components, nature and frequency of adverse events and the patient satisfaction. Material and Methods: Two to three implants of diameters 3.8 and 4.3 mm and lengths of 11 and 13 mm were inserted in the posterior mandible. Opposing dentition were natural teeth or fixed restorations. The implant shoulder was placed at bonelevel. Prosthetic delivery (loading) took place after 6-12 weeks in class I-III or 12-18 weeks in class IV bone. Peri-apical radiographs were taken after surgery and before and after abutment/crown placement. Routine clinical controls, photographs, and peri-apical radiographs were taken at 6, 12, 24, 36, 48 and 60 months after loading. Results: 52 implants were set in 24 patients. Mean age was 48.9 (SD ±13.8). Twenty patients received two, 4 had three neighbouring implants. Thirty-six implants in 17 patients could be followed over 60 months post loading. Seven patients (n=16 implants) dropped out during the course of the study. One patient withdrew consent, two patients died, three patients were lost to follow-up and one patient dropped out by implant failure after 16 months. Two implants got lost equivalent to a survival rate of 95.4%. Between insertion and loading mean bone loss was 0.5 mm (SD: ±0.4) and between loading and 6 months a bone gain of 0.1 mm (SD ± 0.4) was observed. A gain of 0.2 mm (SD: ±0.5) was reported after 24 months and this finding remained stable to the end of the study after 60 months: 0.2 mm (SD: ±0.6). Conclusions and clinical implications: The CONELOG® SCREW-LINE implant is safe and reliable for the partially edentulous patient in the distal mandible with a good performance regarding implant survival, esthetical outcome and bone stability over the course of the study. Background: The platform switching concept refers to the discrepancy between smaller diameter prosthetic abutments related to implant platform diameter. Clinically, the application of such components seems to reduce marginal bone resorption and to maintain soft tissue levels improving treatment outcomes and patient satisfaction. Up to this date the literature is sparse on long-term results deriving from well-designed randomized controlled trials evaluating the efficacy of platform switching versus platform matching. Aim: The purpose of this five-year prospective randomized multicenter study was to compare the clinical performance and peri-implants marginal bone levels of implants restored with platform switching (PS) or platform matching (PM) abutments. Materials and methods: The study enrolled adult patients missing two or more adjacent teeth in the posterior mandible mesially bounded by a natural tooth. Patients underwent open flap implant insertion and were randomly allocated to the PM or PS group, receiving the corresponding healing abutments. Loading was carried out after a healing period of 6-12 weeks (and for bone quality IV 12-18 weeks) with cemented crowns. Patients were followed annually for 5 years. Outcome measures were marginal bone level changes, implant survival and clinical parameters plaque index, sulcus bleeding index and pocket probing depth. Results: Thirty-three patients received 72 implants in the PM group and thirty-five patients received 74 implants in the PS group. Sixty patients attended the final appointment of the study, 31 had received PS components and 29 had received PM components with 65 and 63 implants respectively. Estimated mean difference in marginal bone levels of PS and PM restored implants was 0.28mm (95% CI: [0.06, 0.49], p=0.011), favouring the switching components. After 5 years the overall survival rate was 96.6% with no differences between groups (p=0.647). Conclusion: Patients requiring implant supported restorations in healed bounded or free end edentulous gaps of the mandible benefit from the use of PS prosthetic components from the surgical procedure onwards. Ziel: Die retrospektive Analyse dieser Fallberichte evaluiert Implantatüberleben, Hart- und Weichgewebeveränderungen um OsseoSpeedTM EV Implantate nach periimplantärer Defektaugmentation mit dem alloplastischen Knochenersatzmaterial Symbios^® Biphasic Bone Graft Material (BGM). Material und Methoden: Bei 3 Patienten mit ausgeprägten Alveolarkammdefekten erfolgte die Implantatinsertion mit simultaner oder präoperativer lappenfreier Defektaugmentation mit BGM als alleinigem Augmentat. Unterschiedliche Therapiekonzepte kamen zur Anwendung (Sofortimplantation mit oder ohne Sofortversorgung, Alveolarkammprävention und spätere Implantation). Implantatüberleben, Knochenniveau bzw. -volumen und Weichgewebeparameter wurden nach einem Nachbeobachtungszeitraum von mindestens einem Jahr beurteilt. Im Fall der Spätimplantation konnte eine Biopsie des eingeheilten BGM histologisch untersucht werden. Ergebnisse: Kein Implantat ging im mittleren Nachuntersuchungszeitraum von 21 Monaten verloren. Das mittlere periimplantäre Knochenniveau befand sich auf dem Niveau der Implantatschulter. Das orovestibuläre Knochenvolumen zeigte eine geringgradige Reduktion. Die mittlere Rezession reduzierte sich von 1 auf 0,3 mm. Der Pink Esthetic Score (PES) verbesserte sich im Mittel von 11,3 auf 12,7. Die mittlere Breite der keratinisierten Mukosa blieb unverändert. Die histologische Untersuchung des BGM zeigte nach 4 Monaten eine weitgehende Durchbauung mit Knochen. Zusammenfassung: Das Implantatüberleben, das periimplantäre Knochenniveau und -volumen, die Weichgewebeparameter sowie die schnelle knöcherne Durchbauung lassen vermuten, dass das neue biphasische BGM zur periimplantären Augmentation erfolgreich eingesetzt werden kann. Introduction: Complex defect situations with combined vertical and horizontal dimensions continue to be a challenge even for established augmentation procedures. New computer-assisted augmentation methods with their advantages are particularly suitable for this purpose. In the following, an augmentation of a maxillary anterior region with a combined vertical-horizontal defect which has been treated with a patient-specific CAD/CAM titanium mash based on the DAICOM data (DVT) is presented. Patient case: An otherwise healthy 30-year-old patient presented for the treatment of an endentulous space in the upper front (region 21-23) with a severe horizontal-vertical defect after tumour resection. In addition to the bony defect, there was a significant scarring in the vestibule soft tissue as a result of the pre-surgery. Based on the DICOM data, a patient-specific titanium mesh for particulate augmentation was prepared. Despite dehiscence in the healing phase, the augmentation was successful, and implantation in combination with a soft tissue management using artificial collagen matrix was performed. Die retrospektive Analyse dieser Fallserie evaluiert Implantatüberleben, Hartgewebeveränderungen und Weichgewebeästhetik von sofortinserierten und provisorisch sofortversorgten OsseoSpeedTM Implantaten bei ausgeprägten Rezessionen > 3 mm in der ästhetischen Zone. Material und Methoden: Bei 10 Patienten wurden 12 Implantate als Sofortimplantat in der ästhetischen Zone im Oberkiefer inseriert. Simultan erfolgte eine lappenfreie Augmentation des fazialen Knochendefekts mit autologen Knochenspänen und die Deckung des Rezessionsdefekts mit einem Bindegewebetransplantat sowie eine provisorische Sofortversorgung. Nach mindestens 3 Monaten erfolgte die definitive Versorgung. Implantatüberleben und -erfolg, Knochenniveau und Weichgewebeparameter wurden in einem Nachbeobachtungszeitraum von 1 bis 8 Jahren beurteilt. Ergebnisse: Kein Implantat ging im Nachuntersuchungszeitraum verloren. Das mittlere approximale Knochenniveau betrug bei der finalen Untersuchung -0,3 mm. Die Höhe der fazialen Knochenlamelle betrug präoperativ -7,8 mm und verbesserte sich signifikant auf -0,3 mm. Die Rezession reduzierte sich signifikant im Mittel von 5,1 auf 1,7 mm. Der Pink Esthetic Score verbesserte sich bei allen Implantaten und stieg im Mittel signifikant von 7,2 auf 11,3. Die mittlere Breite der keratinisierten Mukosa erhöhte sich signifikant von 2,2 auf 4,5 mm. Zusammenfassung: Die Reduktion der Rezession, die Regeneration des fazialen Knochenniveaus und die verbesserten ästhetischen Ergebnisse belegen, dass eine Sofortimplantation und -versorgung in Verbindung mit einer autologen Knochen- und Bindegewebeaugmentation in der ästhetischen Zone auch bei ausgeprägten Rezessionsdefekten möglich ist. Trotz der Zunahme von präventiven Maßnahmen wie der Sofortimplantation oder der "Alveolar Socket Preservation" zur Vermeidung atropher Zustände vor geplanten Implantationen sind weiterhin oft Augmentationen vor bzw. während der Implantatinsertion notwendig. Das primäre Ziel jeder Augmentation besteht darin, die Funktion, Prognose und Ästhetik der implantatprothetischen Gesamtversorgung zu verbessern. Neue computerunterstützte Verfahren bieten hierbei die Möglichkeit, bereits etablierte Augmentationsmethoden zu optimieren. Das kommerziell erhältliche patientenspezifische CAD/CAM-gefertigte Titangitter Yxoss erlaubt eine passgenaue und einfache Auflagerungsplastik mit partikulärem Augmentationsmaterial. Der Beitrag beschreibt anhand klinischer Beispiele die monozentrischen Erfahrungen der Autoren mit dem Titangitter und stellt die aktuellen wissenschaftlichen Daten zum partikulären Aufbau mit Titanmeshes vor. Unter dem zunächst wenig aussagekräftigen Titel "Die Zunge" ist in diesem Jahr ein aktueller Farbatlas aber auch zusammenfassende Darstellung der Anatomie und Physiologie der Zunge insbesondere deren physiologischen Variationen und pathologischen Veränderung im Rahmen unterschiedlicher Erkrankungen erschienen.
