Michael G. Newman, DDS, is editor-in-chief of the American Dental Association/Elsevier PracticeUpdate Clinical Dentistry Channel. He is a professor emeritus at the UCLA School of Dentistry, a past president of the American Academy of Periodontology, and a diplomate, American Board of Periodontology. Dr Newman is coeditor of Newman and Carranza’s Clinical Periodontology, 13th Edition (Elsevier, 2019), Newman and Carranza’s Clinical Periodontology for the Dental Hygienist (2020), and Essentials of Newman and Carranza’s Clinical Periodontology (2021). He is founding editor emeritus of The Journal of Evidence-Based Dental Practice.

Implant-supported prostheses often present with mucogingival deficiencies that may cause esthetic or hygienic issues. These issues may present as limited or no keratinized tissue, irregular soft tissue contour or concavity, and gray "showthrough" of the implant abutment and root forms. An interpositional soft tissue graft substitute that generates keratinized tissue and increases soft tissue thickness would be beneficial, as it would reduce donor site morbidity and be available in unlimited, off-the-shelf supply. Thirty patients were assessed as part of a multicenter, practice-based evaluation of the material. A xenogeneic collagen matrix was placed as an interpositional graft on the buccal aspect of implant sites; sites were reassessed at 6 months posttreatment. Results indicated that the collagen matrix increased tissue thickness and keratinized tissue around existing dental implants.
Die Erfolgssicherheit osseointegrierter Dentalimplantate führte zu grundlegenden Veränderungen in der gesamten Zahnheilkunde. Die Chancen, fehlende Zähne durch solche, die hinsichtlich der Funktion und des Aussehens nahezu der natürlichen Bezahnung entsprechen, dauerhaft zu ersetzen, waren zu keinem Zeitpunkt besser. Nach über drei Jahrzehnten, in denen der erfolgreiche klinische Einsatz osseointegrierter Dentalimplantate nachgewiesen wurde, kann heute fundiert festgestellt werden, dass Implantationen zuverlässig erfolgreich sind und Patienten über lange Zeiträume mit funktionstüchtigem Zahnersatz versorgt werden können1-6. Unter Berücksichtigung der vielen Herausforderungen des Mundhöhlenmilieus und der Belastungen durch die einwirkenden Kaukräfte ist dies eine bemerkenswerte Errungenschaft. Der Erfolg der zahnärztlichen Implantologie führte die Zahnheilkunde als Fach insgesamt zu völlig anderen Sicht- und Behandlungsweisen, als dies noch vor 20 Jahren der Fall war.

Clinical and DNA probe analysis were used to evaluate 36 failing implant sites in 13 patients. Failing implants showed evidence of increased mobility and a high incidence of peri-implant radiolucencies in radiographs. The probing depth was greater than 6 mm in 58% of the sites measured. Moderate levels of Actinobacillus actinomycetemcomitans, Bacteroides intennedius, and Bacteroides gingivalis were detected with DNA probe analysis.
