Hintergrund: Die ästhetische Behandlung der Frontzähne ist immer eine besondere Herausforderung. Dank der Fortschritten bei den Dentalmaterialien stehen für diese Behandlung inzwischen vielfältige Möglichkeiten, wie Kompositrestaurationen, Vollkeramikkronen und Keramikveneers zur Verfügung. Die aktuelle Herausforderung in der rekonstruktiven Zahnmedizin lautet: Wie lassen sich hochästhetische Ergebnisse erreichen, während zugleich die biologischen Strukturen so weit wie möglich geschont werden? Mit der Einführung ätzbarer hochfester Keramiken haben Zahnärzte und Zahntechniker heute Materialien und Verfahren zur Hand, die eine minimalinvasive Wiederherstellung der Ästhetik und Funktion erlauben. Mit Vollkeramiken und Adhäsivsystemen der neuesten Generation lässt sich die vorhandene Zahnsubstanz insbesondere bei Einzelzahnversorgungen noch besser erhalten. Ziel: Ziel war die Bewertung der klinischen Bewährung von Lithiumdisilikatveneers mit Federrand. Material und Methoden: Insgesamt 265 Lithiumdisilikatveneers wurden nach Flusssäureätzung und Oberflächensilanisierung mit einem Befestigungskomposit befestigt und von verschiedenen Zahnärzten nachuntersucht. Die Veneers wurden bei 53 Patienten (32 weiblich, 21 männlich) zwischen Oktober 2009 und April 2015 eingesetzt und im Mittel 54,4 Monate nachbeobachtet. Eingesetzt wurden die Veneers in folgender Verteilung: 211 im Oberkiefer (77: zentrale Schneidezähne, 65: laterale Schneidezähne, 49: Eckzähne, 12: erste Prämolaren, 5: zweite Prämolaren) und 54 im Unterkiefer (16: zentrale Schneidezähne, 14: laterale Schneidezähne, 13: Eckzähne, 6: erste Prämolaren, 1: zweiter Prämolar, 2: erste Molaren, 2: zweite Molaren). Die klinische Auswertung basierte auf den modifizierten Kriterien der California Dental Association (CDA) und den Ryge-Kriterien, wozu alle Patienten zwischen Januar und März 2017 wiedereinbestellt wurden. Bewertet wurden folgende klinischen Parameter: Farbpassung, Qualität der Keramikoberfläche, Randverfärbung und Randschluss. Die Daten wurden mittels deskriptiver statistischer Verfahren analysiert. Ergebnisse: Die klinische Überlebensrate der 265 Veneers nach dem Beobachtungszeitraum lag bei 99,63 %. Bei einer Restauration war nach drei Jahren in Funktion infolge eines Traumas ein Adhäsionsversagen eingetreten. Die Farbpassung wurde bei 93,9 % der Restaurationen (248/265) als Alfa und bei 6,4 % (17/265) als Bravo bewertet, und es gab keine Charlie- oder Delta-Wertungen. Die Keramikoberfläche erhielt bei 97,7 % der Veneers (259/265) die Bewertung Alfa und bei 2,2 % (6/265) die Bewertung Bravo. Die Randverfärbung wurde bei 97,3 % (257/265) als Alfa, bei 3 % (8/265) als Bravo und der Randschluss bei 96,9 % (256/265) als Alfa und bei 3,4 % (9/265) als Bravo bewertet. Schlussfolgerung: In dieser retrospektiven Analyse zeigten Lithiumdisilikatveneers mit vertikaler Randpräparation bei den Parametern Farbpassung, Keramikoberfläche, Randverfärbung und Randschluss eine gute klinische Bewährung. Trotz der beobachteten Ergebnisse sind weitere klinische Studien nötig, um die langfristige Bewährung dieser Art der Restauration zu untersuchen.

The esthetic treatment of the anterior teeth has always presented a challenge in clinical practice. With the improvement of dental materials, many restorative options such as resin composites, all-ceramic crowns, and ceramic veneers have become available. The current challenge in reconstructive dentistry is how to obtain excellent esthetic results while preserving the biological structures involved as much as possible. Thanks to the introduction of high-strength etchable dental ceramics, clinicians and technicians now have materials and procedures at their disposal that allow for the restoration of esthetics and function through a minimally invasive approach. New-generation, all-ceramic restorations and adhesive systems allow greater preservation of residual hard tooth structures, especially with regard to single elements. Purpose: To evaluate the clinical performance of lithium disilicate (LiDiSi) veneers with a feather-edge finish line margin. Materials and methods: A total of 265 LiDiSi veneers were cemented with resin cement after fluoridric acid and silane surface treatment and observed by different clinicians. These veneers were placed in 53 (32 female, 21 male) patients between October 2009 and April 2015, with a mean observation period of 54.4 months. Of these, 211 veneers were placed in the maxilla (77 central incisors, 65 lateral incisors, 49 canines, 12 first bicuspids, 5 second bicuspids, 2 first molars, 1 second molar), and 54 in the mandible (16 central incisors, 14 lateral incisors, 13 canines, 6 first bicuspids, 1 second bicuspid, 2 first molars, 2 second molars). The clinical evaluation used the modified California Dental Association (CDA) and Ryge criteria after recalling all patients between January and March 2017. The clinical parameters evaluated were color matching, quality of the ceramic surface, the presence of marginal discoloration, and marginal integrity. The data were analyzed through descriptive statistical analysis. Results: The clinical survival of the 265 veneers over the follow-up period was 99.63%. One restoration showed adhesive failure due to a traumatic event after 3 years of clinical service. The color matching parameter was classified as Alfa in 93.9% (248/265) and Bravo in 6.4% (17/265) of the restorations, with no Charlie or Delta ratings recorded. The ceramic surface was rated as Alfa in 97.7% (259/265) and Bravo in 2.2% (6/265), marginal discoloration as Alfa in 97.3% (257/265) and Bravo in 3% (8/265), and marginal integrity as Alfa in 96.9% (256/265) and Bravo in 3.4% (9/265) of the restorations. Conclusion: In this retrospective analysis, LiDiSi veneers with a vertical finish line showed good clinical performance in terms of color matching, ceramic surface, marginal discoloration, and marginal integrity. Despite the present results, further clinical studies are needed in order to investigate the long-term performance of this type of restoration.

Die ästhetische Behandlung von Frontzähnen war schon immer eine besondere Herausforderung für den Zahnarzt. Aufgrund der Verbesserung zahnärztlicher Materialien sind viele restaurative Optionen wie Kompositrestaurationen, Vollkeramikkronen und Keramikveneers verfügbar. Die aktuelle Herausforderung in der rekonstruktiven Zahnmedizin liegt darin, sowohl exzellente ästhetische Resultate zu erreichen als auch die biologischen Strukturen weitestmöglich zu erhalten. Dank der Einführung hochfester ätzbarer Dentalkeramiken stehen Zahnärzten und Zahntechnikern Materialien und Verfahren zur Verfügung, mit denen sich die Ästhetik und Funktion minimalinvasiv wiederherstellen lassen. Vollkeramische Restaurationen und Adhäsivsysteme der neuesten Generation ermöglichen besonders bei Einzelzahnrestaurationen einen größeren Erhalt der vorhandenen Zahnhartsubstanz. In diesem Artikel wird eine tangentiale Präparationstechnik für Keramikveneers beschrieben.

Die Kommunikation mit dem Patienten und innerhalb des Behandlungsteams ist ein kritischer Faktor, der insbesondere bei komplexen und interdisziplinären Behandlungsansätzen das Ergebnis beeinflussen kann. Bei einer erfolgreichen Kommunikation sollte nicht das Marketing im Mittelpunkt stehen, sondern die korrekte Erfassung der intraoralen Situation des Patienten. Auf diese Art wird die Behandlung besser akzeptiert und die Zufriedenheit des Patienten mit der Therapie gefördert. Eine verbesserte Kommunikation innerhalb des Behandlungsteams ist ein wesentlicher Faktor für ein besseres definitives Ergebnis, das zudem auf effiziente Weise erreicht wird. Mittlerweile sind neue technische Geräte wie Tablet-Computer weit verbreitet. Mit ihrer Hilfe und klinischen Bilddaten kann die Kommunikation mit dem Patienten visuell und interaktiv gestaltet und der Austausch innerhalb des zahnärztlichen Behandlungsteams verbessert werden. Es wird eine faziale, dentolabiale und ästhetische Analyse des Patienten erstellt, die in den verschiedenen Schritten des klinischen Behandlungsverfahrens genutzt werden kann. Damit wird die Vorhersagbarkeit des ästhetischen Ergebnisses erhöht und die Anzahl der notwendigen Behandlungssitzungen reduziert. (Int J Esthet Dent 2014;9:28-40)

Communication with the patient and within the team is a critical factor that can influence the treatment outcome, especially in complex and multidisciplinary dental treatments. Indeed, effective communication, not focused on marketing but on proper information of the patient's intraoral situation, can encourage greater acceptance of treatment and also greater treatment satisfaction. Better communication within the dental team is also a very important factor to improve the final result and reduce the time needed to reach it. Thanks to new technological devices widely available, such as tablets, we can use visual communication and interaction with the clinical images of the patient in order to improve communication with the patient, and especially within the dental team. The use of this method allows us to obtain a facial, dentolabial and dental esthetic analysis of the patient that can be used in various clinical steps, improving the predictability of the esthetic outcome and at the same time reducing the number of clinical sessions usually required.
